Application Instructions

Applications are accepted year-round, remain active for one full year, and may be renewed by phone or in writing. They are available on-line in Adobe PDF Format or at the Service Center.

FILLING OUT THE ON-LINE APPLICATION DOCUMENT
          [Please note that the application form is not interactive, it will not automatically be saved.
          How you choose to save a copy depends on your system set-up and tools.]
Three options (followed by tips for options 2 and 3):
     1) Print the document, then complete it using a typewriter/word processor
     2) Complete pages 1-3 on-line prior to printing, then complete page 4 and consent form by hand
     3) Save the document, fill out pages 1-3, print, then complete page 4 and consent form by hand
Tips:
     • Use the zoom feature on your computer to enlarge the form for ease in completing
     • To move forward between fields use the Tab key or point and click with the mouse
     • To move backward between fields use Shift Tab or point and click with the mouse
     • To check (uncheck) boxes toggle with the space bar or point and click with the mouse
Most fields are not limited to a specific number of characters. Type size will reduce on lengthier-than-normal responses to fit into the space provided. Please be sure that responses are readable. In fields with limited space, you might fill in as much as is easily read, then complete your response by word processor.
