    Chaffin Online Registration

    To enroll a student at Chaffin Jr. High, please read thoroughly and follow carefully the instructions below:

    1. Gather the required documents before you begin:
      1. Official birth certificate (NOT hospital birth certificate)
      2. Social security card
      3. Up-to-date immunization record
      4. proof of residency - current utility bill showing name of guardian along with service address
      5. Custody agreement (if applicable)
      6. Parent photo ID
    2. Go to the Fort Smith Public Schools website to fill out the online registration form.  You may access it by clicking on Enroll Your Student under the Families tab at the top of this page. Note: The address you enter needs to be the address where the student actually resides. Please be accurate when indicating who the student lives with. If you are not legally married to your significant other, they may not be listed as a parent or step-parent, but that person may be listed as the Emergency Contact. Please provide all current phone numbers where parents can be reached for emergencies and disciplinary issues. The home phone number must be a home phone number or personal cell, not a work phone number.
    3. Fill out the following additional registration forms. They are available as fillable PDF documents. Do this by downloading each document first. Then, fill it out and save it. Once you have completed them, you will return them by email to: dgreen@fortsmithschools.org
      Subject Line: Registration Documents
      Note: Your registration will not be complete until these documents are returned.
      1. Fill out the Authorized Pickup Persons form to let us know who is authorized to pick up your student from school. Anyone not listed here will not be allowed to check out your student.
      2. Fill out the Request for Records form that grants us permission to request records from the current school.
      3. Complete the Home Language Usage Survey.
      4. Fill out the Race and Ethnicity form.
      5. Complete the following forms: Title VI ED 506 Indian Student Eligibility and Military Dependent form.  If one or both of these do not apply then leave them blank.
      6. Fill out the Employment Questionnaire.  This is a required form.
    4. Watch for an email from Chaffin. This email will let you know we have everything we need to complete your registration or what we still need in order to complete your registration.
    5. Once the online registration is complete and we have all required documents, you will be contacted by one of the counselors to discuss the selection of courses.

  • Title I Program

    Beginning in 2018-19, Chaffin Junior High School will participate in a school-wide Title I program.  The Title I program is a federally funded program which provides supplemental educational services to ensure that all children have a fair, equal, and significant opportunity to obtain a high quality education.  It also helps to ensure that students reach, at minimum, proficiency on challenging state academic achievement standards and state academic assessments.

    The Fort Smith Public School District is governed by a seven-member Board of Education.
    Four members are elected by zone while three are elected in at-large positions.


    The FSPS BOE will meet at 5:30 p.m. on March 8, 2021 for a called board meeting.

    The FSPS BOE will meet at 5:30 on Monday, March 29, 2021 for a regularly scheduled board meeting.

    The meetings will take place at the Service Center Building 'B' Auditorium, 3205 Jenny Lind Road, as well as virtually for board members and FSPS staff.
