The School-Based Health Center (SBHC) is a collaborative effort between Fort Smith Public Schools, Mercy Clinic, and The Guidance Center. Our goal is to provide exceptional mental and physical healthcare to all FSPS staff, students, and their families. By offering healthcare services directly at school, the SBHC ensures that students feel their best, enabling them to excel academically. With the convenience of on-site care, students miss less class time, and parents can stay at work knowing their children are receiving the attention they need. It's a win-win for everyone!
At the SBHC, we are committed to fostering a healthy school environment where every student has access to quality healthcare and support. By promoting wellness and providing healthcare directly on campus, we aim to help students succeed in school and beyond, ensuring a healthier future for all.
How it Works

Complete and Sign Registration Forms: Parents or guardians must complete and sign registration forms before their student can access SBHC care. It is recommended to complete these forms in advance to ensure prompt service if your student becomes ill.

Return Completed Forms: Completed and signed forms can be delivered to the student's school or emailed to our confidential email address. Each packet includes two Release of Information forms - one for the primary care physician (if applicable) and one for the school nurse. Registration forms must be renewed annually.

Accessing Services: If a student falls ill during school hours, they can access the SBHC through the school nurse. Family members and staff can schedule appointments by contacting the SBHC at 479-784-8131. The SBHC encourages parental involvement in healthcare decisions and welcomes any questions or concerns.

Advisory Board
The SBHC Advisory Board assists in integrating SBHC services within the community and advises on policy and program matters. Comprising of parents, teachers, school administrators, business representatives, health professionals, and concerned citizens, the advisory board supports efforts to enhance students' medical, social, emotional, and behavioral health. Additionally, the board works to address the impacts of poverty and other challenges faced by students, staff, and families.
